We have digitized patient forms and placed them on our Web site for the convenience of our patients. This ensures we have complete information we can access quickly and that helps us serve you better. Please read the information below before proceeding.
- You can use your computer to type in the requested information using the free software package Adobe Acrobat Reader. If you do not have this software, download it now at no cost.
- Adobe Acrobat Reader will not allow you to save information you type into this form, so you will want to print the form if you choose to complete it electronically. If you have the complete version of Adobe Acrobat, you may be able to save the information to your local hard drive. OR, you may choose to simply print the blank form, completing it manually, offline.
- After completing the form electronically, you print the form from your computer and bring it with you to your appointment. OR, bring the manually completed form with you to your appointment. OR, you can complete the form at our office.
- When the form requests a signature, please sign the printout; you cannot type in your name on signature lines.
- Whether you choose to complete the form electronically or fill in the blanks witha pen, remember to bring it with you to your appointment.
Do you need?
2009 Patient Form should be completed by all patients, new and current, before your first/next appointment with one of our medical professionals. This includes our Privacy Notice (if you have not viewed our privacy notice, please do so now), insurance and payment information, missed appointments information, release of medical information, release of insurance information, and the disclosure of private medical information.
Privacy Notice. You will be asked on our New Patient Form to acknowledge that you have read and understand our Privacy Notice; please take a moment to read it. You also may want to retain a copy for your records.
